2005 "Larry Weber" ALS Achievement Award
Criteria
An individual who has advocated the advancement of pre-hospital care and made significant
contributions to EMS in his/her community or the state of North Dakota.
Intent
To recognize those who have significantly contributed to local and state provision, development,
and improvement of pre-hospital care and EMS education in their communities. The award is to
reflect both past and present contributions.
Deadline
Postmarked by March 1, 2005

Nominations must be submitted using the official nomination sheet. If more space is needed, please
attach the necessary information. Award nominations must be received as a complete package. If
desired, supplemental items such as photographs and newspaper articles may be included.
The award will be presented during the banquet at the ND EMS Association annual Spring Conference
held in April at Bismarck. Nominee should be present at the banquet to accept the award in the
event he/she is named. (Advanced notice will be provided to the nominating person/agency.)
